More than 118,000 voters were left off the voting rolls in Los Angeles County for Tuesday's primary, officials said.
California Secretary of State Alex Padilla has asked for a detail report to explain the errors, the Los Angeles Times reported. The county's Board of Supervisors has called for an investigation.
About 2 percent of voters were affected by the error at 35 percent of voting locations, the board said.
Election officials became aware of the problem Tuesday and said in a statement the problem was due to a printing error.
"A total of 1,530 precincts of the County's 4,357 voting locations were affected in some way, with a total of 118,522 voters' names omitted," officials said.
People whose names were not on the list were given a provisional ballot.
"We fell short in meeting the more than reasonable expectations of the voters and poll workers," County Clerk Dean C. Logan said Wednesday. "It's an issue that I take seriously, I understand the gravity of it, especially in an environment where there is so much discussion about the security and integrity of our voting process."
Logan's office said 95 to 90 percent of provisional ballots are usually determined to be valid and are counted in voting numbers.
One of the names left off the list was actor Henry Winkler.
"My name was left off the polling registry today in Los Angeles," Winkler tweeted on Tuesday.
